Are you ready to become a patient and a member of Madsen Medical Direct Care? Or maybe you’d just like to start with an initial assessment and consultation? Great! Here is what you can expect:
Fill out the enrollment form by clicking here. Even if you’d like to start with an assessment, we still ask that you fill out the enrollment form so we can collect all necessary information. Please note that a credit or debit card is required to complete enrollment.
We will contact you to schedule your initial assessment and consultation once Dr. Madsen and Trisha, CNP review your information and determine if Madsen Medical would be a good fit for you.
The initial assessment and consultation with Dr. Madsen is $100. During your 45-minute assessment Dr. Madsen will take a detailed medical history, examine you, and get to know you and your health goals. If you decide to continue your care with Madsen Medical, this $100 fee will count as your first month’s payment.
Before leaving the office at your assessment and consultation, we will schedule any necessary follow up appointments or testing, collect payment, and confirm that your automatic monthly payment of $100 is set up. If you’d prefer to pay $1,100 annually instead of $1,200 annually when billed monthly, we can take that payment at that time.
